Death or injury to baby/fetus by failure to diagnose or manage
antenatal problems such
as maternal diabetes, antibody formation,
intra-uterine infection, intra-uterine growth
retardation, pre-eclampsia, twins, abnormal and/or
unstable presentations, fetal
abnormality (including genetic testing), fetal
head-pelvic disproportion, breech position,
risk of placental abruption, placenta praevia,
premature labour

Death or injury to baby/fetus by failure to diagnose or manage
problems during labour
such as malpresentation of the fetus, disproportion
and shoulder dystocia, umbilical
cord complications, ruptured placenta, abnormal
changes in the fetal heart rate,
ruptured uterus

Injury to mother by failure to diagnose and adequately repair
tear and manage pre-
eclampsia
·
Failure of sterilisation
·
Perforation of uterus following introduction of intra-uterine
contraceptive devices
·
Failure to interpret or act appropriately upon cervical
cytology
·
Damage to posterior abdominal wall, gastro-intestinal tract,
bladder and/or ureters
during surgery
·
Inadvertent sterilisation by introduction of infection
·
Failure to remove swabs and other artefacts
·
Delay in the diagnosis of pregnancy, ectopic pregnancy and
genital cancer
